Israel trailed Ukraine near the end of the third quarter but went on a decisive 16-0 run in route to an 81-73 victory in the additional qualifying tournament for EuroBasket 2007.
Having won this tournament two years ago, the Israelis know what it takes to grab the last remaining place for the EuroBasket.
Ukraine came into the contest with a new coach.
Valentin Berestnev replaced Vitaliy Lebedintsev, who stepped down after the team's 70-45 capitulation to Belgium on Wednesday - the worst defeat in the country's history.
The Ukrainians battled in Tel Aviv and led 57-56 with 17 seconds to go in the third.
Shooting guard Oleksandr Kolchenko had scored 21 of his game-high 30 points by that stage.
Ukraine then failed to score for more than four minutes.
Israel went on their decisive surge during that dry spell with Lior Elihayu, Yotam Halperin and Yaniv Green doing the bulk of the damage that left that hosts in command at 72-57.
Ukraine got no closer than seven points the rest of the way.
Eliyahu had 20 points to pace Israel.
Halperin added 18 and Green weighed in with 16.
